Meet Dr. Sahni

Dr. Nishant Sahni, MD

Nishant Sahni, MD, MS

Board-Certified Internal Medicine
Mayo Clinic Faculty University of Minnesota Faculty 20+ Years Immunology MS — UT Southwestern
"After 20 years as a hospitalist, I've seen the true cost of metabolic disease — not just the medical bills, but the lost decades with family and the dreams unfulfilled. I started Cadence Health because GLP-1 medications are transformative, but only when paired with real physician oversight. This isn't about losing 30 pounds. It's about gaining 10 healthy years."

Dr. Sahni personally manages every patient — ordering labs, interpreting results, adjusting doses, and answering questions. No rotating providers. No call centers. One physician who knows your history, your labs, and your goals.

Common Questions

FDA-approved Wegovy and Zepbound are manufactured to pharmaceutical-grade standards and backed by rigorous clinical trials. Compounded versions are mixed by third-party pharmacies without FDA oversight for safety, purity, or potency. The FDA has found impurity levels up to 86% in some compounded products and has issued over 100 warning letters. Cadence Health prescribes only FDA-approved, brand-name medications shipped directly from the manufacturer.
GLP-1 medications affect multiple organ systems — metabolism, cardiovascular function, pancreas, kidneys, and thyroid. Proper management requires ordering and interpreting labs, titrating doses based on individual response, and proactively managing side effects. A board-certified Internal Medicine physician has the training and clinical experience to manage these effects safely. At Cadence Health, Dr. Sahni personally manages every patient — not a rotating roster of providers.
Our $99/month membership is a cash-pay service. For the medications themselves, coverage varies by plan. Many insurers now cover Wegovy and Zepbound with prior authorization. If your insurance covers the medication, your cost may be as low as $25/month with a savings card. If not covered, the manufacturer cash-pay programs (NovoCare for Wegovy, LillyDirect for Zepbound) offer significantly reduced pricing — often 60-70% off list price.
Clinical trials demonstrate 15-22% body weight loss over 12-18 months with proper medical supervision (STEP and SURMOUNT trials). Most patients notice appetite changes within the first few weeks, with steady progress as doses are titrated up. Individual results vary based on starting weight, medication choice, adherence, and lifestyle factors. Dr. Sahni will optimize your treatment based on your individual response and lab results.
We order comprehensive baseline and follow-up labs including: A1c and fasting glucose (diabetes prevention), comprehensive metabolic panel (kidney and liver function), lipid panel (cardiovascular risk), and TSH (thyroid — important for GLP-1 safety). Additional markers like hsCRP, HOMA-IR, and body composition may be tracked based on your individual risk factors. All labs are ordered by Dr. Sahni and personally interpreted — not just flagged by an algorithm.
Yes. Every patient receives a GLP-1 nutrition guide focused on high-protein intake to preserve muscle mass during weight loss. We also refer you to a registered dietitian through our partner network — and for most patients, these visits are covered by insurance at no out-of-pocket cost. Nutrition counseling is critical during GLP-1 therapy to optimize results, prevent muscle loss, and build sustainable habits for after you reach your goal weight.
GLP-1 medications are FDA-approved for adults with BMI ≥ 30, or BMI ≥ 27 with at least one weight-related condition (prediabetes, high blood pressure, high cholesterol, sleep apnea). You must be a Minnesota resident (we'll expand to additional states via IMLC). Cadence Health is not appropriate for patients with a personal or family history of medullary thyroid cancer or MEN2 syndrome, or those who are pregnant or planning pregnancy.
